The Department of Health (DOH) does not require you to get an exemption form before receiving the COVID-19 vaccine. The COVID-19 vaccination has no exemption papers available from DOH. Only parents/guardians who want to exempt their kid from the vaccines necessary for children in K–12 schools, preschools, or daycare may do so by obtaining a Washington State Certificate of Exemption (COE). The COE does not include the COVID-19 vaccination because it is not currently required in Washington for kids to go to daycare or school.
